LPA observed at 2:49pm, there was a knock at the door, the assist open, closed the door, walked over to the day-care area in the family room picked up C2, walked back to the front door opened the door handed C2 to mother, closed the door, walked back to the day-care area/family room, obtained C2's blanket, and backpack, walked back to the front door and hand the mother C2's belongings, while the mother waited outside the door This poses an immediate risk to health, safety or personnel rights of persons in care.
