Teacher Child Ratio: There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement was not met as evidenced by staff interviews and review of files revealed there were two aides supervising up to 13 children at the time of the incident. Teacher #1 stated she went to the restroom and when she returned to the playground area a few minutes later.
