Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on observation, interview, record review, the licensee did not comply with the section cited above in staff (S4) did not have a current mandated reporter training certificate (expired on 11/27/2025), which poses a potential health, safety or personal rights risk to persons in care. Director acknowldeged the certificate was not curent.

This requirement was not met as evidence by Interviwes conducted, LPA interviewed multiple former and current staff. LPA discovered a pattern of behavior from S1, and one staff observed S1 punish and yell at children as a away to make an example of the child in front of other children. This is a potential health and sfaety risk to persons in care,

In particlualry it was found C8 was not allowed to go to the bathroom by S1. And S1 took food away from children.
