(a) The licensee shall provide care and supervision as necessary to meet the children’s needs.(1) No child shall be left without the supervision of a teacher at any time… Based on observation, interview, and record review, the licensee failed to ensure that a child was provided care and supervision at all times. On 2/24/26 at approximately 10:42 AM, an infant was found alone inside the classroom crying and visibly upset. Staff confirmed that the child had been left unsupervised in the classroom for approximately 10 minutes during a transition to the outdoor area.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidenced by: A gas leak/smell in the infant classroom that persisted for approximately 4 to 6 weeks before it was resolved with the intervention of the gas company. Any gas leak should be immediately reported to Gas company. This poses a potential risk to health and safety of children in care.
