Such structural changes shall include, but not be limited to, room additions. This requirement is not met as evidenced by: Based on observation, interview, and inspections on 12/05/23 and 4/24/25, the licensee did not comply with the section cited above as the facility did not notify the department that they lost control of Rooms 606 and 608. Facility also did not notify the department that they will use Room #4 that is not approved for School age program. This poses an immediate health, safety or personal rights risk to persons in care.
