Out of the five children's files reviewed, it was noted that two children were missing the Identification and Emergency Information form, or that the parent/authorized representative had not signed it. Additionally, one child did not have any of the following licensing forms signed by the parent/authorized representative: LIC 700, LIC 627, LIC 613A, LIC 995A. Not having a complete record of children poses or posed a potential health, safety, or personal rights risk to persons in care.
