This requirement is not met as evidenced by: Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 7 staff files reviewed, which poses an immediate health, safety or personal rights risk to persons in care. LPA notes one staff (S4) did not have a criminal record clearance nor an association to the facility. S4 was not listed on the Guardian roster as of 11/10/2025.
