The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement was not met as evidenced by: C1 sustained an injury on their back that staff was unable to determine how the injury occurred. The injury resembled fingerprint marks. This is an immediate risk to the health and safety of children in care.
The licensee shall comply with all terms and conditions set forth in the admission agreement. This requirement was not met as evidenced by: Center Management staff terminated enrollment of C1 which is not in accordance with the parent handbook. This is a potential risk the health and safety of children in care.
To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not met as evidenced by: Surveilane footage shows S1 pointing their finger in C1's face multiple times, pushing C1 to an area of the classroom and aggressively grabbing C1 by the arm to redirect and reprimand them. This was seen more than four times and is an immediate risk to the health and safety of children in care.
A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met as evidenced by: During the course of the investigation LPA learned school age children and preschool children were being commingled . This poses a potential health and safety risk to children in care.
