Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department ...within the Department's next working day... a written submitted... within 7 days... This requirement was not met as evidenced by: LPA learned a child, although never left fully, visually unsupervised by a staff, did leave the facility. This was not reported to CCLD within 24 hours nor was a UIR submitted within 7 days. This poses a potential health, safety, and/ or personal rights risk to persons in care
