101238.2 (g) Outdoor Activity Space. The playground shall be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence shall be at least four feet high. This requirement was not met as evidenced by: Based on the LPA’s observation, the temporary green fencing was not attached to the wooden pole which could allow access to the off-limit area where hazardous items were observed. The director failed to ensure the playground was in good repair before allowing children and staff access. This is a potential health and safety risk to children in care.
