This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above in 56 out of 56 persons in that LPA observed adult scissors on a counter in Room 5; a basket on a table containing several containers of sunscreen, scissors on a cubbie and a mop with an attachment containing cleaning solution in Room 7 which poses a potential health, safety or personal rights risk to persons in care.

On 04/05/2022 at 4:27 PM, child #1 was left unsupervised behind the enclosed play yard gate for an undetermined amount of time until staff noticed the gate door was left opened and the latch broken. The staff went to retrieve child #1 behind the gate. This poses an immediate health and safety risk to children in care.
