(d) Upon the occurrence…of any of the events…below, a report shall be made to the Department…by telephone or fax within the…next working day...In addition, a written report…shall be submitted to the Department within seven days...(1) Events reported shall include...(E) Epidemic outbreaks. This requirement was not met as evidenced by: Based on Director and Licensee’s admission, epidemic outbreak was not report to CCLD reported within 24 hours or in writing within 7 days from the initial occurrence. Based submission of multiple Unusual Incident Reports on 1/20/22, it was determined between 1/6/22 -1/19/22. Director did not report an epidemic outbreak to CCLD within 7 days. This poses a potential risk to children in care.
