Responsibility for Providing Care and Supervision for Infants (B) Staff shall physically check on sleeping infant(s) every 15 minutes and document the following: This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in sleep logs are only being ocmpleted for infants up to 1 years of age which poses/posed a potential health, safety or personal rights risk to persons in care.
STAFF-INFANT RATIO. There shall be a ratio of one teacher for every four infants in attendance. This requirement was not met as evidenced by: Based on evidence gathered, it is determined that the facility operated out of ratio on a few ocassion with 2 teachers and 10 infants.
Infant Needs and Services Plan (b) The needs and services plan shall be in writing and shall include the following: (2) Infants up to 12 months of age shall have a completed Individual Infant Sleeping Plan [LIC 9227 (3/20)], which is incorporated by reference. This requirement is not met as evidenced by: Based on records review and interview with Director, no children under 12 months had completed Infant Sleep Plans (LIC9227) which poses a potential health, safety or personal rights risk to infants in care.
