This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above in that furniture cushions located in outdoor play are are torn and have exposed foam and batting which poses/posed a potential health, safety or personal rights risk to persons in care.

Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in that 4 out of 4 staff files did not have a current mandated reporter training certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Records...All personnel records shall be maintained at the child care center and shall be available to the licensing agency for review...The licensee may keep such records in a central administrative location provided that they are readily available to the Department at the child care center site...This requirement has been met as evidenced by: Based on interview with the Director and observation of her conversation on the phone with the District office, the District refused to allow the documents off site. LPAs were unable to verify immunizations, criminal record clearances, transcripst and other documents that, if absent, constitutes a potential risk to the health and safety of children in care.
