PERSONAL RIGHTS To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by: Based on interview and document reviewed, a 3-year old child was taken to emergency and received stitches. A fish tank that was placed on top of play refrigerator fell on top of child. It was noted that the refrigerator was not safely braced to the wall. LPAs also observed several shelves and aquariums that were not anchored to the wall.

This requirement was not met as evidenced by: Based on record review, licensee failed to associate a staff to the facility. It is noted that this staff is a volunteer who works more than 16 hours per week.

Teacher qualification & duties - The units specified in (c)(1) above shall include courses that cover the general areas of child growth and development, or human growth and development; child, family and community, or child and family; and program/curriculum. This requirement is not met as evidenced by: Based on LPA's review of facility records, 1:6 teachers did not complete the required education to be a teacher. This poses a potential health & safety risk to children in care.
