This is an amended version of the original report created on 03/03/2026 101229(a) The licensee shall provide care and supervision... (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met as evidenced by: Based on documentation, video footage and interviews, the facility staff did not comply with the section cited above, as a child was left unattended in the classroom for approximately 2 minutes before the director found the child in the classroom. This poses an immediate health, safety, and personal rights risk to persons in care.
(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interviews conducted, record review and video footage review, the licensee did not comply with the section cited above when a child was left alone in the bathroom which poses an immediate health and safety risk to children in care.
