CCR 101216.2(e) (e) An aide shall work only under the direct supervision of a teacher. Site Supervisor will create a plan on how they will ensure a qualified teacher is present, at all times when children are in care. Site supervisor will conduct a meeting and provide LPA with a copy of the agenda which includes aides only working under the supervision of a teacher. Based on video footage, staff interviews and site supervisor admintance 3 teachers aides were left alone with 7 children in care without a qualified teacher present. Site Supervisor will provide with information to the LPA by 1/24/2025.
Responsibility for Providing Care and Supervision for Infants (B) Staff shall physically check on sleeping infant(s) every 15 minutes and document the following: This requirement is not met as evidenced by: Based on analyst interview and record review, the Site Supervisor did not comply with the section cited above as the sleep documentation every 15 minutes does not include the required components which poses/posed a potential health, safety or personal rights risk to the child in care.
