Buildings and Grounds (g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. This requirement is not met as evidenced by: Based on observation, laundry room was unlocked and had laundry detergent accessible to children. Room 2 backdoor on right, was unlocked and lead to unlicensed adjacent recreation room/classroom for Boys and Girls Club. This poses a potential hazard to the health and safety of children in care if not corrected.

The licensee shall provide care and supervision as necessary to meet the children's needs...No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interview and record review, Child #1 was left behind on the playground during a transition into the classroom, for approximately 1-2 minutes. This is a potential, rather than immediate hazard to the health and safety of children in care as Child #1 was without supervision, in a contained area, for no more than 2 minutes.
