Responsibility for Providing Care and Supervision for Infants (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. Under no circumstances shall ANY infant be left unattended. This requirement is not met as evidenced by: Based on interviews and record review, the licensee did not comply with the section cited above. On 2/24/2025 two infants napping in the crib area were without visual supervision when 5 staff and 10 infants evacuated when the fire alarm went off. This deficiency poses an immediate health, safety or personal rights risk to persons in care.
