This requirement was not met as evidenced by: Based on interviews with staff, child-on-child bitings occured when staffs' attention was directed toward the child requiring additional support and adequate supervision of other children was not maintaine which is a potential health, safety and personal rights risk to children in care.

Responsibility for Providing Care and Supervision for Infants (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. Under no circumstances shall ANY infant be left unattended. This requirement is not met as evidenced by: Based on interviews and record review, the licensee did not comply with the section cited above. On 2/24/2025 two infants napping in the crib area were without visual supervision when 5 staff and 10 infants evacuated when the fire alarm went off. This deficiency poses an immediate health, safety or personal rights risk to persons in care.
