101229 (a)(1) The licensee shall provide care and supervision... (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in that on March 16, 2026, there was a lack of supervision resulting in seven children being left unsupervised in classroom #2 which poses an immediate, safety, or personal rights risk to persons in care.

(h) (1) .... the department may permit an individual to transfer ...criminal record clearance, ..., from one facility to another, ...transferred to another facility licensed... shall be submitted to the department on a form provided... or submitted ...clearance that can be transferred. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above by not associating one staff member to the facility that has been employed for the past 8 months which poses a potential, safety, or personal rights risk to persons in care.
