(1) Any injury suffered by a child in care shall be reported to that child's parents... regardless of treatment by a medical professional. This requirement is not met as evidenced by: Based on interviews the Licensee did not comply with the section cited above by not notifying the child's parents of an injury that occured on 06/06/2025, which posed a potential threat to the health and safety of children in care.

This requirement is not met as evidenced by: Based on Licensee admittance and interview, the licensee did not comply with the section cited above. The Licensee stated she recieved notification that a child who was injured in care received medical treatment and it was not reported to the Department. This is a potential risk to the health, safety or personal rights risk to persons in care.
