This requirement was not met as evidenced by: Based on licensee’s admission and records reviewed, licensee failed to submit a written unusual incident report about a significant lack of supervision incident that occurred on 02/10/2022, when three children walked out of the day care facility unsupervised, which poses an immediate Health and Safety risk to children in care.
