The requirement was not met as evidenced by, Based on 2 of 4 children identified as potential witnesses stated that Staff #1 (S1) told a child that they would place tape over their mouth if they didn't stop talking and 3 of 5 staff members identified as potential witnesses stated they have witnessed or overheard S1 talk to the children in an aggressive tone and make inappropriate comments to children. This is a potential health and safety risk to children in care.

Written Directives per AB2370: A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidenced by: Based on water testing results and interviews, facility tested over the Action Level Exceedance level at one faucet in Room A. This posed a potential health, safety, or personal rights risk to children in care.
