This requirement is not met as evidenced by: Based on interview and record review, the site supervisor, Mr. Lautanen, has been in the position since January 2026, without the licensee notifiying the Department of the change or submitting the required Director's Packet for review, which poses a potential health, safety or personal rights risk to persons in care.
