This requirement was not met as evidenced by: Based on investigation interviews and relevant documents reviewed it is determined that child C1 sustained an unexplained injury at the facility on 10/24/24, due to a lack of supervision. Investigation interviews determine staff S1 observed the child without injury at 5:06pm but observed the child with injury by the time the child was picked up which poses/posed an immediate health, safety or personal rights risk to children in care.
