101229 Responsibility for Providing Care and Supervision (a)(1) - The licensee shall provide care and supervision ...No child(ren) shall be left without the supervision of a teacher at any time... The requirement was not met as evidenced by: Based on interviews and record reviews, a lack of supervision occurred for Child #1 (C1) during an 11/05/24 self-reported incident at the facility when he left his classroom by going through a propped open back door into a secured playground and was not discovered as missing for anywhere from four to six minutes by another classroom teacher coming on to the playground which posed an immediate Health and Safety risk to the child in care.

Lead Action Level at Child Care Centers “ … “Licensees shall maintain a lead value at or below the Action Level of 5 ppb in all outlets subject to the testing requirements of these Written Directives, for the health and safety of children in care.” This requirement is not met as evidenced by: On 08/31/2022, Lic. did not maintain a lead value at or below the Action Level for water lead testing resulting with values of 5.5 ppb or greater for the exterior water fountain. Water testing results identified with Action Level Exceedance (ALE) as defined in WD section 101700.3. are not deemed safe to drink. Based on record review, the Lic. did not comply with the above directive; the exterior water fountain tested with an ALE. This poses as a potential Health & Safety risk to children in care.
