Reporting Requirements: (d) Upon the occurrence….. any of the events specified in (d)(1) below, a report shall be made to the Department by telephone…. by the next working day and a written report containing the information specified in (d)(2) below shall be submitted...within seven days following the occurrence of such event. (B) Any injury to any child that requires medical treatment. This requirement was not met as evidenced by: Based on record review, and interviews, C1 obtained medical treatment due to an injury that occurred at the CCC. CCL was not notified via telephone and was notified in writing, twenty days after medical attention was obtained. This failure to provide timely notification poses a potential risk to the health, safety, and personal rights of children in care.
Infant Care Activities (A) Staff shall place infants up to 12-months of age on their backs for sleeping. (4) Infants with an Individual Infant Sleeping Plan [LIC 9227 (3/20)] that have Section C of the form completed and signed by an authorized representative shall be placed on their back when first laid down to sleep. In the event the infant changes position, the infant may remain in the alternative position. (a) Upon staff observation that the infant is capable of rolling from their back to their stomach and stomach to their back, the provider shall fill out Section D of the Individual Infant Sleeping Plan [LIC 9227 (3/20)], notify the authorized representative, and obtain the authorized representative's signature on the individual sleeping plan no later than the next business day following observation. This requirement is not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited above in two of five child fiels audited. Where C1 and C2 LIC 9227 had sect: C and D blank but were developmentally rolling over. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Personal Rights: (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful, and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by: Based on interviews and records reviewed, it was revealed that staff allowed infant aged children to go down a slide face first. This is not a safe or developmentally appropriate activity for infant aged children. This poses a potential health, safety, or personal rights risk to persons in care.
Indoor Activity Space for Infants: (b) Indoor activity space for infants shall be physically separate from space used by children in the child care center and school-age child care center components. This requirement was not met as evidenced by: Based on interviews and record review, commingling occurred when previous staff school age children were left unattended in the infant classroom and playground on two separate occasions with daycare children present.

Limitations on Capacity: (a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met as evidenced by: Based on interview and record review, school age child was commingling with infant age children on July 29, 2024. CCC does not have a school-age license and school age child was left unattended by parent.
Child's Medical Assessments (a) Prior to, or within 30 calendar days following the enrollment of a child, the licensee shall obtain a written medical assessment of the child. This medical assessment enables the licensee to assess whether the center can provide necessary health-related services to the child.
