Outdoor Activity Space- (d)The surface of the outdoor activity space shall be maintained: (2)Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement was not met as evidenced by: The Director fenced off the storm drain area on 1/3/24 and made the Upper Pre-K outdoor play area free of hazards for children in care by making it inaccessible to children and submitted proof to the dept. on 1/24/24. Based on observation and interviews there was an accessible storm drain at the top of a slope on the playground, approximately 2 ft. deep that created a potential safety hazard to children in care playing around that area. This posed a potential risk to the health and safety of children in care.
