Modifications to Infant Needs and Services Plan (a) The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in 6 out of 7 children's records which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights - (a)The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by: Based on staff interviews the licensee did not make sure the correct baby bottle was given to the correct infant. This poses a potential risk to the health and safety of children in care.
Staff-Infant Ratio - (b) There shall be a ratio of one teacher for every four infants in attendance. This requirement was not met as evidenced by: Based on observation and staff interviews, the Infant room ratio was observed to be 2:9, nine infants supervised with two staff. This poses a potential risk to the health and safety of children in care.
Responsibility for Providing Care and Supervision for Infants In addition to Section 101229, the following shall apply: (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not met as evidenced by: Based on Staff interview, on 9/30/22 a one year old child crawled out of his classroom and was unsupervised for approximately 37 seconds. This poses a potential health and safety risk to children in care.
