License number; use in advertisements; publications, or announcements (a) Each child day care facility licensed under this chapter, Chapter 3.5 (commencing with Section 1596.90), or Chapter 3.6 (commencing with Section 1597.30) shall reveal its license number in all advertisements, publications, or announcements made with the intent to attract clients. This requirement was not met as evidenced by: Based on observation and staff interviews conducted by LPA, licensee has signage and a website meant to attract clients that does not contain their facility number. This poses a potential health, safety or personal rights risk to the children in care.
Licensure Requirements Every licensed child day care center shall have one or more carbon monoxide detectors in the facility that meet the standards established in Chapter 8 (commencing with Section 13260) of Part 2 of Division 12. The department shall account for the presence of these detectors during inspections. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. The licensee does not have an operational carbon monoxide detector. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. Sign In and Sign Out (b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in 8 out of 24 children present are not signed in by a parent/guardian. This poses a potential health, safety or personal rights risk to persons in care.
Infant Care Teacher Qualifications and Duties: Prior to employment, an infant care teacher shall have completed, with passing grades, at least three postsecondary semesters or equivalent quarter units in early childhood education or child development, and three postsecondary semester or equivalent quarter units related to the care of infants, at an accredited or approved college or university. This requirement is not met as evidence by LPA Siao's observation. Licensee Anita Okolo was observed to be supervising the three toddlers in care today and she is in the process of completing the infant course. This poses a potential health and safety risk to clients in care.
Criminal Record Clearance All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 101170(f) This requirement is not met as evidence by S1 and S2 was working at the faciltiy prior to their background clearance being associated to the facility. S1 is no longer an employee at the facility and S2 worked at the faciltiy for a couple months prior to being associated to the facility. Civil penalty of $1000 is being assessed today. This poses a potential health and safety risk to clients in care.
Teacher Qualifications and Duties: Prior to employment, a teacher shall meet the requirements of (b)(1) or (b)(2) below: (1) A teacher shall have completed, with passing grades, at least six postsecondary semester or equivalent quarter units of the education requirement specified in (c)(1) below, or shall have obtained a Child Development Assistance Permit issued by the California Commission on Teacher Credentialing. This requirement is not met as the two year old staff Maria Montenegro and teacher aide Yensenia Robles does not have the required units to be teachers and they were observed alone superiving the children during today's inspection. This poses a potential health and safety risk to clients in care.
