Documentation shall include the following: a. Date. b. Infant’s name. c. Time of each 15-minute check. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above, as she did not possess and maintain the safe sleep logs for 3 of 3 infants in care, which poses a potential health, safety or personal rights risk to persons in care.
