THIS IS AN AMENDED VERSION OF AN ORIGINAL REPORT. Criminal Record Clearance. All individuals subject to a criminal record review... shall prior to ...residing...in a licensed facility...Obtain a California clearance or a criminal record exemption... This requirement is not met as evidenced by: Based on conversation with Licensee, adult resident Mikayla Hatzopoulos, has been living in the home since the latter part of 2020. She has a fingerprint clearance with a different agency but not an active clearance with Community Care Licensing. This is a potential hazard to children in care and a $500 civil penalty will be issued on for LIC 421
