Immunizations (g) The licensee shall document each child's immunizations and shall maintain such documentation in the center for as long as the child is enrolled. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in one out of five children's files did not have documentation of immunizations or proper documentation for immunization exemption (C#3) which poses/posed a potential health, safety or personal rights risk to persons in care. LPA is noting that facility did have email communication with San Diego Public Health and a copy of Individual Education Plan (IEP) in the file stating the IEP will suffice as proof of child being exempt from immunizations.
