(d) Upon the occurrence, during the operation of the child care center of any of the events specified in...(1)Events reported shall include the following:(B)Any injury to any child that requires medical treatment. This requirement was not met as evidenced by, Based on interview, it was disclosed to LPA that a UIR was not completed for C1 injury which required parent to seek medical treatment. This is a potential risk to the health and safety of children in care.
Admission Procedures and Parental and Authorized Representative's Rights: (2) Conduct one or more personal interviews with the child's parent or authorized representative that meets the following requirements: (B) Provides the child's parent or authorized representative with information about the child care center that shall at least include procedures to be followed .... ...should the child become ill or injured while at the child care center, and procedures for conducting inspections for illness. This requirement was not met as evidenced by, based on interviews conducted parent was not notified until pick up of childs injury nor was a UIR submitted to CCL. This is a potential risk to the health and safety of children in care.
California Lead Action Level at Child Care Centers(b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an : Action Level Exceedance. This requirement was not met as evidenced by, based on records review and interviews, facility tested over the Action Level Exceedance at 12, in one faucet in school-age classroom. This poses a potential health, safety, or personal rights risk to the children in care.
Limitations on Capacity: A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This was not met as evidenced by, based on observation and interview, only one classroom is currently being used for school-age with low enrollment numbers and the other school-age classrooms are being used for the Adult Day Program.
