- Outdoor Activity Space (d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. LPA observed several holes on the surface of the play area that poses a tripping hazard. This condition poses a potential health, safety or personal rights risk to persons in care.

This regulation requirement was not met as evidenced by: Based on Mr. Nguyen statement regarding the incident of 3/16/2022, it was determined that staff failed to provide sufficient visual supervision to two children who admitted to the school principal that they were engaged in an inappropriate activity in the bathroom.
