Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time... This requirement was not met as evidenced by... Based on facility self report that C1 walked away from the group during a classroom transitioning and exited the facility. C1 exited the front door and was located 100 yards away in the apartment complex. This poses an immediate risk to the health and safety of the child.
