Safety precautions shall include but not limited to: (3) Where children are less than five years old are in care, stairs shall be fenced or barricaded. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above in that, upon entry the barricade that prevented access to the stairs was not engaged and there were children present that were under the age of 5. This poses/posed a potential health, safety or personal rights risk to persons in care.

Safety precautions shall include but not limited to: (4) Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children. (A) Storage areas for poisons, firearms and other dangerous weapons shall be locked. This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above in there were 6 total items that stated "call poison control" on their label. These items were inaccessible to children in a latched cabinet underneath the kitchen sink. However, these were not secured by a key lock or combination lock. This poses/posed a potential health, safety or personal rights risk to persons in care.

- (a) The licensee shall be present in the home& ensure children in care are supervised at all times. When circumstances require licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. This was not met as evidenced by: video footage showing licensee leaving the property during the time children were in care. This poses a potential risk to the health and safety of children.
