This requirement is not met as evidenced by: Based on observation, interview and records review, the licensee did not comply with the section cited above in 1 out of 2 staff records reviewed. Staff #2 did not have proof of MMR and Tdap immunizations on file for review, which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds 101238(a) The childcare center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidenced by: Based on LPA observation the licensee did not comply with the section cited above in that LPA observed the following areas with debris: toilets, sink, gates, carpets, tables, chairs, and floors which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Center Sleeping Equipment 101439(b) - A crib or portable-crib meeting United States Consumer Product Safety Commission safety standards shall be provided for each infant who is unable to climb out of a crib. This requirement is not met as evidenced by: Based on observation the licensee did not comply with the section cited above in that at 1:34PM, LPA observed a non-walking/crawling infant sleeping on a pillow round on the carpet which poses/posed an immediate health, safety or personal rights risk to persons in care.
