The provider shall supervise infants while they are sleeping and adhere to the following requirements: The provider shall physically check on the infant every 15 minutes. This requirement is not met as evidenced by: Based on observation and interview, the licensee did not comply with the section cited above in that there were 2 out of 3 infants present (Child 4 [C4] & Child 5 [C5]) that did not have 15 minute safe sleep logs completed. Additionally, Child 1 (C1)'s last 15 minute safe sleep log was completed in June of 2025 and has no other more recent 15 minute safe sleep logs available to review, which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Based on interviews, the licensee did not comply with the section cited above in that Licensee confirmed she was unaware of time and place of the incident between two day care children that involved inappropriate behavior, which posed an immediate health and safety risk to persons in care.
