Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review and interview, the licensee did not comply with the section cited above in that 1 out of 5 staff, Staff 1 (S1), had a mandated reporter training certificate that expired 5/11/25 while today's date is 5/28/2025, therefore the renewal of mandated reporter training has not been completed within 2 years. This poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Based on observation the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed a bleach bottle in a cabinet that was not locked; however, the cabinet was inacessible to children. LPA observed no children were in the area where the cabinet was.

Criminal Record Clearance: All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 101170(f) Based on record review the licensee did not comply with the section cited above. Staff was providing care and supervision to children without being associated to the facilities license(s). This poses/posed an immediate health, safety or personal rights risk to persons in care.

When no longer needed by the child...all medications shall be returned to the child's authorized representative or disposed of after an attempt to reach the authorized representative. This requirement was not met as evidenced by: Nonprescription medication which is no longer needed by Child 1 after 9/3/2021 is stored at the facility. As stated by administrator Kris Thewes, the medication was only needed for one day on 9/3/2021. Storing no longer needed medication in the facility presents potential risks to the child's health and safety.

This requirement was not met as evidenced by: Health screening report for Staff #2 and #3, and TB test report for Staff #3 were not available for review during inspection. Administrator Kris Thewes stated that they could not locate the records at the time.
