Report finding: Child's Medical Assessments (a) Prior to, or within 30 calendar days following the enrollment of a child, the licensee shall obtain a written medical assessment of the child. This medical assessment enables the licensee to assess whether the center can provide necessary health-related services to the child. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in that 2/5 children's files, Child 3 [C3] & Child 4 [C4], did not contain a written medical assessment. These files also did not contain an appointment date for the medical assessment of the children or a tuberculosis test within 30 days of the children's admission. This poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by: Based on interview and record review, the Licensee did not meet the above regulation which poses an immediate health risk to the children in care. The facility self-reported a child being observed with a vape pen in their possession and believed the vape pen was in the child’s mouth.
