Licensees shall maintain a lead value at or below the Action Level of 5 ppb in all outlets subject to the testing requirements of these Written Directives, for the health and safety of children in care. This requirement is not met as evidence by: Based on record review of required lead testing, the facility had lead values of 5 or above on water outlets: A,B, and D from 12/2025 and 01/2026 retests. This is a potential health and safety risk to persons in care.

Outdoor Activity Space (g) The playground shall be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence shall be at least four feet high.
