- (1) No child(ren) shall be left without the supervision of a teacher at any time.... Supervision shall include visual observation. This requirement is not met as evidenced by: Based on records review, interview, and self-report from the facility, the facility did not maintain visual observation of 1 child, for about 2 minutes, while the child was on the facility playground, with the playground door propped open. This poses/posed an immediate health, safety or personal rights risk to persons in care.
