101229 (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... This was not met as evidenced by: Based on staff's statements and self reported incident, on 03/02/2026 a child was left unattended in the classroom for a period of 10-15 minutes during transition to outdoor play. This poses/posed an immediate health/safety and personal rights risk to children in care.

Fixtures, Furniture, Equipment and Supplies (n) Furniture and equipment shall be maintained in good condition, free of sharp, loose or pointed parts. This requirement is not met as evidenced by: Based on LPA, observation of outdoor playstructure, the licensee did not comply with the section cited above. At 01:30pm, LPA observed the bottom step of facility's play structure's protective coating peeling/missing with exposed metal underneath starting to rust. The peeling material may pose a tripping hazard and potential health, safety or personal rights risk to persons in care.
