This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in that 3/5 children's records reviewed (Child 3 [C3], Child 4 [C4], Child 5 [C5]) did not contain the verification of receipt of licensing reports (LIC9224) for the Type A citations issued on the dates of: 7/10/24, 8/29/24, 9/17/24 (2 reports on this date), and 11/14/25. These children did not need these documents completed while they were enrolled in the infant program, however they did need them completed after transitioning to the preschool program during the year of 2025. This poses/posed a potential health, safety or personal rights risk to persons in care.
