Responsibility for Providing Care and Supervision for Infants (B) Staff shall physically check on sleeping infant(s) every 15 minutes and document the following: (3) Infants up to 12 months of age who are sleeping in a position other than on their back. (a) If the infant’s Individual Infant Sleeping Plan [LIC 9227 (3/20)] does not have Section C completed, staff shall return the infant to their back for sleeping. This requirement is not met as evidenced by: Based on observation and record review, the facility did not comply with the above regulation as LPAs observed three infants, without Individual Infant Sleeping Plans (LIC9227) on file, to be sleeping on their stomachs. Due to there being no LIC9227s on file, LPAs could not verify that children could roll of their own accord and that they were not placed on their stomachs by staff which poses an immediate health, safety or personal rights risk to persons in care.
