Safety precautions shall include but not limited to: (1) Fireplaces and open face heaters shall be screened to prevent access by children. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. LPAs observed that the home was not clean including floors were dirty and covered in dog hair, kitchen had soiled pots and pans out along with garbage on the counters, outdoor area smelled like dog urine and toys and outdoor equipment were dusty and dirty. There is a screen attached to the door that leads to the backyard that is frayed and unsafe for children which poses/posed a potential health, safety or personal rights risk to persons in care.
