This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. LPA's observed during the visit children playing near the sliding door in the playground to the right of the building. LPA Deanna Villagrana opened the sliding door and did not see a teacher present. LPA went around the corner and observed the teacher sitting in a chair to right of the building with no view towards the sliding door. Children were not being supervised which poses an immediate health, safety or personal rights risk to persons in care.

Angela obtained an influenza waiver for staff who were present. Teacher 2 in missing influenza vaccine which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by during an investigation for a complaint filed on 01/24/2020, it was observed that former director Adrianna Castello was antagonizing a child when she held the child's toy over a garbage can threatening to throw it away. Child was observed crying. Adrianna called the child a stupid spoiled little brat. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.
