101229 Responsibility for Providing Care and Supervision (a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...shall include visual observation. Director states they have revisited the revised behavior plan with all staff, following the incident on 8/26/24. Director will conduct a training on supervision and line of site with all staff memebrs. Director will submit training material and sign in sheet to LPA by 4pm on 9/13/24. This requirement is not met as evidenced by: based on interview and record review, Facility did not comply with the section cited above as children were able to play under a sheet without direct supervison, which poses an immediate health, safety, or personal rights risk to persons in care.
