Outdoor Activity Space (1) Provide a shaded rest area for the children. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA learned that the facility has been utilizing infant outdoor area since September 2022.

Outdoor Activity Space (e) As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed artificial grass under the play structures. This material is not one of the approved materials listed in the regulations. This material needed approval from CCL prior to installation.
Child's Records (b) Each record shall contain information including, but not limited to, the following: (8) Medical assessment, including ambulatory status as specified in Section 101220, and the following health information: This requirement is not met as evidenced by: Based on a record review the licensee did not comply with the section cited above in 1 out of 4 files reviewed did not contain a phsycians medical assessment which poses/posed a potential health, safety or personal rights risk to persons in care.
The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement is not met as evidenced: Staff #1 mistakenly gave Child #1 hospital grade disinfect to drink instead of water. This is an immidiate risk to the health and safety of chidlren in care.
